This application is intended for graduate students only (e.g. Doctoral, Ed. Specialist, Masters and Graduate Certificate students)

Enter your name as you would like it to appear on your diploma (completion of this application will not update your name with the university. To officially update your name, please contact the Office of the University Registrar, 125 Jesse Hall or check out this link.
Do not use nicknames, quotation marks or parentheses in your name.
You may use hyphens and accent marks to accurately reflect your name.
Providing this information will enable us to cross check your file for graduation review and will enable us to print your name in the commencement program (provided you do not have a privacy hold).
